What type of confined space is involved?

Which hazards are present or potential in this space?

Select all that apply. The presence of any of these hazards classifies the space as a Permit-Required Confined Space (PRCS) under OSHA 1910.146(b), requiring a written permit before entry.

Atmospheric testing and ventilation controls

Atmospheric testing is the most critical pre-entry step under OSHA 1910.146. Answer each question for your current entry practice.
Is atmospheric testing conducted before every entry and continuously during occupancy?
OSHA 1910.146(c)(5)(ii) requires testing in the order: oxygen, flammable gases, then toxic air contaminants
Is atmospheric testing equipment calibrated and maintained per manufacturer specifications?
Uncalibrated meters give false readings and are a direct cause of confined space fatalities
Is forced air ventilation used when atmospheric hazards are present or possible?
OSHA 1910.146(c)(5)(ii)(E): ventilation must be continuous if needed to maintain safe atmosphere
Are all energy sources (electrical, mechanical, pneumatic) isolated and locked out before entry?
LOTO under 1910.147 must be completed before confined space entry when energised equipment is present

Permit program and training status

OSHA 1910.146 requires a written permit program, trained authorised entrants, attendants, and entry supervisors, and a documented entry permit for every permit-required entry.
Does your facility have a written Permit-Required Confined Space program?
Required under 1910.146(c) for any employer whose workers enter permit-required confined spaces
Is a written entry permit completed and posted before each permit-required entry?
The permit must list: space, purpose, date, authorised entrants, attendants, hazards, controls, communication, equipment, and rescue procedures
Is a trained attendant stationed outside the space for the entire duration of entry?
The attendant must never enter the space, must maintain communication, and must initiate rescue if needed per 1910.146(i)
Have all authorised entrants, attendants, and entry supervisors received OSHA-compliant confined space training?
1910.146(g) requires training before assignment and when duties or hazards change
Is a non-entry rescue system (retrieval line, tripod, winch) in place and tested before each entry?
Non-entry rescue is required where feasible; sending rescuers in without equipment causes most confined space rescue fatalities
Has there been a confined space incident, near-miss, or OSHA citation at this facility in the past 3 years?
Prior confined space incidents indicate systemic program failures that remain unaddressed
