Which fall protection controls are in place?

Fall protection program and training status

Answer each question based on your actual worksite. OSHA 1926.503 requires training for all workers exposed to fall hazards, and 1926.502 requires a written fall protection plan for certain operations.
Is there a written site-specific Fall Protection Plan?
Required for leading edge, precast concrete, and residential construction per 1926.502(k)
Have all exposed workers received OSHA-compliant fall protection training?
OSHA 1926.503 requires training on fall hazard recognition and use of fall protection systems
Are fall protection systems (harnesses, anchors, guardrails) inspected before each use?
OSHA requires pre-use inspection; damaged equipment must be removed from service immediately
Are anchor points engineered to support at least 5,000 lbs per attached worker?
OSHA 1926.502(d)(15) requires anchors capable of supporting 5,000 lbs or designed by a qualified person
Is there a documented rescue plan for workers who fall and are suspended in a harness?
Suspension trauma (harness hang syndrome) can be fatal within minutes; OSHA expects a prompt rescue plan
Is there a competent person on site who can identify fall hazards and has authority to correct them?
OSHA 1926.32(f) defines a competent person as someone with knowledge to identify hazards and authority to take corrective action
Has there been a fall-related incident, near-miss, or OSHA citation at this site in the past 2 years?
Prior fall incidents are the strongest predictor of future falls; they indicate systemic hazards remain unaddressed
