Workplace heat risk factors

Answer Yes or No for each condition. These are the primary heat stress risk factors identified in OSHA 3154.
Are any workers new, returning after absence, or not yet acclimatised to heat?
OSHA 3154: lack of acclimatisation is the leading cause of heat fatalities in the first days of work
Do workers wear heavy PPE, impermeable suits, or multiple clothing layers?
Impermeable clothing prevents sweat evaporation, significantly increasing core body temperature
Is free cool drinking water available at all times (at least 1 cup every 20 minutes)?
OSHA recommends water, rest, and shade. Dehydration accelerates heat illness onset
Is shaded or cooled rest area available within close reach of all workers?
Access to shade or cool rest areas is a core OSHA 3154 engineering control requirement
Have there been any heat illness cases or near-misses at this site in the past year?
Past heat illness incidents are the strongest single predictor of future risk at a worksite
Is there a written Heat Illness Prevention Plan with assigned first aid responsibilities?
OSHA requires a written plan covering water, rest, shade, acclimatisation, and emergency response
Have supervisors and workers been trained to recognise and respond to heat illness?
